Seven Steps Of Care Management:

1. Family Consultation - Our Certified Care Manager conducts a consultaion by phone with the senior and or their family. 
2. Assessment - An in-home comprehensvie in-depth assessment of home safety, physical, mental, social, emotional, and fiancial strengths and weaknessess, and functional abilities. 
3. Holistic Living Plan - After all the data is put together after the in-home assessment, a roadmap for the near term and future is put together identifying options for the senior and their family. 
4. Implementation - Our Certified Care Manager will implement the options that you select in the living plan by referring the best care resources in the community.  Then oversee their work, making arrangements for services, coordinating, facilitating and implementing to bring about positive changes in the quality of life for the senior and their family.
5. Monitoring - We monitor the senior's well-being, programs and services for appropriateness, quality and satisfaction.  We assure that the senior does not receive less than the need or pay more than they require. 
6. Re-evaluation - Life is a transitional process.  We stand by our clients throughout these changes, and are prepared to reassess their needs and help them adapt to the changes which will surely come. 
7. Advocacy -  Our Certified Care Manager advocates for the senior to maximize well being, dignity, and quality of life. She acts as a family surrogate for children living remotely.

Fall Prevention & Home Safety Evaluations: 

A comprehensive fall prevention and home safety evaluation that encompassess all of the home. 

Counseling and Emotional Support Services: 

In-person counseling and support services are provided by our partner, Family Behavorial Health.

Medication Management:

Comprehensive assessment of medication management.  We work closely with the various prescribing physicians to assure proper medication management and patient compliance.
